Report All Liability Claims (Injuries to non-employees) Immediately

Please call and report the liability claim to GB Services at (800) 780-9561. It is recommended that the Liability Claim Report Form be completed before calling GB Services. The Liability Claim Report Form can be found on the Insurance office’s web page under Insurance/Forms and Documents. Much of the information that GB Services will ask for is on this report. GB Services will give you a report number and that number should be written down on your report. Keep this report in your files for future reference.
